Care Plans, Continuing Care and the Decision Support Tool (DST)

For care plans of people who we consider are eligible for continuing care, we base our care plans on the domanins of the DST. Therefore in Domain 1 - Behaviour, we would look for any problems in that area and then identify the management of those problems and how this is to be evaluated.

Residents going to hospital without an escort.

Care services do not have a legal obligation to provide an escort for a resident being taken to hospital. In some cases one is provided, if the family or legal representatives agree to pay for this cost. However, where an escort is not provided, members of the East Midlands Ambulance Service (EMAS)  have at times refused to take the resident to hospital.  This is not an agreed policy within EMAS and they have no right to do this. People have the same right to be taken to hospital from a care home, just as someone living in their own home (without an escort). LinCA has made representation to EMAS, but there has been little response. The Executive Board is raising this again. In the meantime we are issuing a letter, which you may wish to use.  If we are consistent in our approach, we may have a bigger impact.
